Billing for Waiting Time

I'm working on implementing a system for a client. Fortunately, I've been able to meet the deadlines for all of my deliverables. Now, the client is pushing back go-live dates and also falling behind on their own deadlines (servers not ready, requirements not defined). This is extending the duration but not the effort required for the engagement.

My question is, at what point do you other small business owners push back and start charging clients for being too slow? If it continues like this, a project that was initially 6 months will take 9 months, which spreads my top line thinner over time.
